If you are coming in for the first time, here are a few things we suggest to make the visit more productive..
Please ensure that all your pertinent medical records have been forwarded to our office from your referring physician. You may have them fax it to 913 - 451 8575 (attention Lorrie). Sometimes it is better to hand-carry MRI, CAT scan or Ultrasound films. Usually we can review the films and return it to you the same day.
Do try and fill out completely the questionnare we mailed you when you made the appointment.If you want, you can do this online by filling out the i-health record. Also do feel free to jot down questions or concerns that you would like to address at the time of your appointment.
Diabetic patients are requested to bring their glucometers if possible.
If you want your appointment sooner than scheduled, please request your referring physician's office to call us - we will try and accomodate you as soon as possible.We really work hard to speed up new patient appointments.
